                             Assistive Device Program

                 Ministry of Health and Long-Term Care

The Assistive Device Program covers 75% of 3 sets of garments per year for lymphedema patients.  Here are the steps you need to take:

1. You must be assessed by a physician who specializes in the area of vascular surgery, medical or radiation oncology.  This assessment is required every 2 years or if the medical condition has changed.  This doctor will begin the process of filling out the application form. 

2. I am an ADP authorizer and can assess you and sign the ADP form

3. You will be referred to an ADP certified fitter to be measured for compression garment or sleeve
